So what exactly is RPM?

RPM allows patients with chronic conditions to use connected mobile devices and technology to gather and transmit their biometric health data to their PCP’s via secure care management interface (smartphone app or cellular device). This patient data can then be analyzed in real time for timely clinical interventions or reviewed upon next office visit for better disease management.

Along with the basic 3 codes, shown on right, RPM program includes myriad of codes related to virtual visits, interprofessional consultations etc. which can increase realized revenue from $125 to $300 per patient per month.
