Both AWV and CCM programs are excellent opportunities to promote compliance with multiple quality measures and facilitate documentation on illness burden of a patient for an improved and accurate RAF score. All of our programs offer the unique capability of integrating client customizable quality measures, so that our staff works toward closing these care gaps during wellness visits or care coordination activities.

Even though our pdf reports will suffice the need for billing a claim, we do understand that certain clients need discrete data entered into their EMR's for better reporting capabilities. We work with our clients to accommodate these objectives by designing processes that are mutually beneficial and time efficient.
